Most of our BH:Students activities require registration. If you need to register for an activity this is where you can get plugged in! You can check here for information about our trips, overnight activities, book clubs, really everything we do that requires you to sign up!
If you need to register for a Student Life activity that you are attending through BH:Students a Student Life Waiver Form is required.
Brook Hills MRF: A Brook Hills Medical Release Form is required for ANY off-campus event with the Brook Hills Student Ministry. Once you’ve completed a form, you’re good for a year. If you would like to find out if your form is still up to date,
Mission Release Forms
All those over the age of 16 who work with minors at The Church at Brook Hills are required to complete a DHR Background Check. If you are interested in serving with Students in any way, please download this form and return to the Student Ministry Office when it's completed.
