All those over the age of 16* who work with minors in any way (Preschool, Children, or Students) at The Church at Brook Hills are required to complete the following volunteer application process. If you have any questions, please contact .

*If you are under 16, please see a leader in the area of ministry you are interested in serving.

Step 1: Select an Application Based on Your Age

Please print and complete the application corresponding to your age. The application for 19 and older includes a DHR background check.

Complete if you are 18 and under

Step 2: Select Ministry Policy Handbook

Please read and review the policy handbook below corresponding to the ministry you will be serving in, and sign the signature page.

Complete to volunteer with preschool and children

Step 3: Submit Paperwork

Turn in the hard copy of the signed application and the signature page from the policy handbook to your ministry leader. Ink signatures only. Digital signatures will not be accepted.
