If you are a participant in the Impact New Members Worskshop, assess yourself, and ask two other Christ-followers who know you well (spouse, family member, friend, etc.) to assess you. You should enter your name, email address and date of birth. However, you can leave the field below "Assessment Completed by:" blank.
If you are assessing a participant in the Impact New Members Workshop, please enter their name and email address in the corresponding fields. If you do not know their date of birth, you may leave that field blank. Then, enter your name in the field below "Assessment Completed by:". NOTE: THE PARTICIPANT WILL RECEIVE A COPY OF YOUR ASSESSMENT IN THEIR EMAIL. If you would rather them not, simply enter your email address in the "Email" field and we will make their top three gifts according to your assessment available to them and keep the rest confidential.
